REQUEST FOR QUALIFICATIONS
The Carbon County Board of Commissioners is seeking a firm (Vendor)
to prepare an architectural feasibility study for the expansion of the
Carbon County Sheriff's Office and Jail located at 240 West Main
Street, Price, Utah.
The feasibility study will include an assessment of the building by
an architect skilled in correctional facilities. The assessment will
include drawings of the building as it stands as well as drawings of
the proposed expansion, an analysis of needs and expansion scenarios
to accommodate those needs, future growth estimates, detailed cost
estimates and alternate energy or energy efficient options.

Sealed proposal documents are due by 5:00 p.m. November 19, 2012.

A list of all architects or architectural firms who have timely
submitted sealed proposals will be read into the record at a public
meeting of the Carbon County Board of Commissioners on November 20,
2012. However, the proposals will not be opened or reviewed at that
Carbon County is not responsible for proposals that are received late
due to delays in delivery by any source. Faxed proposals are not
accepted. Carbon County reserves the right to reject any and all
proposals, to waive any informality of any proposal and to select the
proposal which in the opinion of the County provides the best product,
warranty and service to the County.

GUIDELINES AND QUALIFICATION STATEMENT PREPARATION
Vendor's submittal shall be as set forth below. The Vendor will
confine its submission to those matters sufficient to define its
qualifications and to provide an adequate basis for Carbon County's
evaluation of the Vendor's qualifications.
Vendor's proposal in response to this RFQ will be incorporated into
the final agreement between Carbon County and the selected Vendor.
The submissions must include but are not limited to each of the

2. Project Deliverables, including Timeline
3. Project Management Outline
4. Detailed and Itemized Cost
5. Appendix: References
6. Appendix: Company Overview and Staffing
The detailed requirements for each of the above-mentioned sections
are outlined below.
DETAILED SUBMITTAL REQUIREMENTS
This section will present a high-level synopsis of the Vendor's
response to the RFQ. The Executive Summary should be a brief overview
of the engagement and should identify the main features and benefits
of the vendor's qualifications. Please respond to the following
questions about your company and your experience:
Do you have experience preparing architectural feasibility studies?
Please describe the information you would include in this feasibility
Do you have experience working with correctional facilities?
Include descriptions of the types of reports that will be used to
summarize the various phases of the work and provide a timeline for
delivery of said reports.
PROJECT MANAGEMENT OUTLINE
Include the method and approach used to manage the overall project
and County's collaboration. Outline in detail how the engagement
proceeds from beginning to end.
DETAILED AND ITEMIZED PRICING
Include a fee breakdown by project phase and estimates of travel
expenses and number of inspections. Include payment schedule based
upon scope of work, deliverables and reimbursables.
Provide three current client references for which you have performed
Submit letter-sized drawings and photographs of previous remodel,
expansion work your firm has been involved in involving correctional
APPENDIX: COMPANY OVERVIEW AND STAFFING
Provide the following for your company:
Official registered name (Corporate, D.B.A., Partnership, etc.),
address and telephone number.
Key contact name, title, address (if different from above address)
telephone number and email address.
Person authorized to contractually bind the organization for any
proposal against this RFQ.
Brief history, including year established and number of years your
company has been conducting similar work.
Include relevant experience and qualifications of key staff and
management personnel that would be assigned to this project.
Evidence that the company has an architect licensed in the State of
EVALUATION FACTORS FOR AWARD
The following elements will be the primary considerations in
evaluating all submitted proposals and in the selection of a Vendor:
1. Completion of all required responses;
2. The extent to which Vendor's proposed solution fulfills Carbon
County's stated requirements as set out in this RFQ;
3. An assessment of the Vendor's ability to deliver the indicated
service in accordance with the specifications set out in this RFQ; and
4. The Vendor's stability, experience and record of past performance
in delivering such services.
Carbon County, may at its discretion and without explanation to the
prospective Vendors, at any time choose to discontinue this RFQ
without obligation to such prospective Vendors.
Carbon County reserves the right to:
Reject any and all submissions and discontinue this RFQ process
without obligation or liability to any potential Vendor; and
Award a contract on the basis of negotiated services, taking into
consideration discussions or requests for best and final offers.
