in this Court, and adjudication may be made involving YOUR PARENTAL
RIGHTS TO N.D. and L.D. You  are hereby summoned to appear before this
Court in Carbon County, Utah, located at 149 EAST 100 SOUTH, PRICE,
UTAH 84501 on January 26, 2011, at 2:00 p.m. for a Trial involving the
permanent termination of your parental rights to N.D. and L.D. You
have the right to be represented by counsel in these proceedings. 

If you fail to appear in person at the Hearing, your parental rights
to the child may be permanently terminated by the Court without
further notice to you.
