THE ABOVE-NAMED CHILD. You are hereby summoned to appear before this
Court in Carbon County, Utah, located at 149 EAST 100 SOUTH, PRICE,
UTAH 84501, on October 6, 2010, at 1:45 p.m. You have the right to be
represented by counsel in these proceedings.

	If you fail to appear in person at the Hearing, your parental rights
