As members of Carbon and Emery counties, the staff at Rocky Mountain Care is committed to delivering compassionate and professional health care services to all those who entrust our staff with their health care needs.

The Rocky Mountain Home Care staff is dedicated to helping patients meet their healthcare needs at home. Patients are more comfortable and heal more quickly in their own personal environment.

Under the direction of Home Care Director of Clinical Services; Heidi Yoklavich, the home health staff assists local community members to heal and rehabilitate following life changing medical events.

When a patient faces a life-limiting illness, Rocky Mountain's Hospice staff is able to assist. Patients who are diagnosed with a terminal disease may seek help from the hospice. The main goal of the hospice staff is to offer the patient, their family and caregivers medical, emotional and spiritual support while obtaining the best quality of life possible during the final stages of the disease. Rocky Mountain Care's Hospice Director of Clinical Support is April Barrett.

The staff at Rocky Mountain Care is familiar with the Carbon and Emery county areas. We have served the communities for 15 years and still going strong. We realize that many community members cannot afford to have proper medical treatment. Therefore, the Rocky Mountain Care Foundation was formed.

The foundation is funded through private donations, grants and volunteer services. We are excited to be partners with the United Way of Southeastern Utah. Through this partnership, patients who have no other means to obtain medical equipment and supplies will have access to needed materials that will allow them to live independently while receiving proper medical care.

Also through this partnership, patients and their families will receive social interaction and support through a volunteer program. This relieves caregivers by providing some respite.

The Rocky Mountain Care Foundation has provided care for some patients who do not have any insurance or financial resources. By partnering with the United Way of Southeastern Utah, we have been able to achieve a greater quality of life for several members of the local community.
